To apply for C/O form AJ, businesses will have to prepare a number of documents as follows:
- Application for issuance of C/O form AJ has been completed completely;

- Notarized copy of customs declaration with completed customs procedures. If the goods are exported and do not require a customs declaration according to regulations, a copy of this document will not be required;
- Notarized copy of commercial invoice;
- Notarized copy of bill of lading or equivalent transport document if the enterprise does not have a bill of lading;
- Calculation of regional value content (compared with regional value content criteria);
- Declaration of HS codes of input and output materials of the product;
- Notarized copy of the goods production process;
- Notarized copy of declaration for import of raw materials and spare parts for production if the enterprise imports raw materials for production;
