What is the AK form? Co Form AK - Certificate of Origin Form AK is a common certificate of origin for goods import and export goods from Vietnam to Korea based on AKFTA Agreement.
The AK form not only proves the origin of goods but also helps businesses reduce import taxes. Co Form AK applies to goods exported to Korea and other countries in the ASEAN region that are entitled to tax incentives as prescribed.

Content on the form of AK form
The main content in the form of AK form includes:
- Boxes 1 and 2: Write information about exporters (Exporter) and imporer (Importer);

Regulations on agencies and time of C/O issuance
Agency: Import -export management departments of the Ministry of Industry and Trade and the Management Board of Export Processing Zone - Industrial Park are authorized by the Ministry of Industry and Trade to issue C/O Form AK.
C/O issuing time regulations:
- Time to receive documents: morning 7:30 - 11:00 pm, afternoon 13h30 - 16h00;
- Time to return records: Morning 8:00 am - 11h30, afternoon 14:00 - 16h30;
- Goods export goods: C/O Form AK issuance time shall not exceed 04 working hours (from the time of receiving complete and valid dossiers);
- Sea export goods and other means: C/O Form AK issuance time shall not exceed 08 working hours (from the time of receiving complete and valid dossiers;
- Goods exported by post: C/O Form AK issuance time is 01 working day from the date of record on the envelope.
In case of refusal to grant C/O Form Ak?
Some reasons may be denied C/O Form Ak:
- The person who proposed C/O Form AK has not yet registered for a trader file;
- The person who proposes C/O Form AK has not yet submitted the debt documents from the previous C/O issuance;
- The person who proposed C/O Form AK does not provide sufficient archives as prescribed for the C/O organization to conduct post -inspections;
- The person who proposed the C/O form AK has a fraudulent manifestation of origin from and the C/O level before and until that time has not been resolved;
- Dossiers of request for C/O Form AK in contravention of regulations;
- Application for C/O contradictory content;
- There are clear grounds to prove that the goods applying for C/O without origin according to the law;

How is the procedure for applying for form AK?
To apply for a paper form, you need to register for a trader file, prepare the application:
Register for a trader profile
If the organization is CO, the person who offers the first level to submit:
- Register the signature sample of the authorized person to sign an application for the certificate of AK form and the seal of the trader;
- Business registration certificate of traders (copies);
- Certificate of tax code registration;
- List of production facilities of traders.
Application for form AK
To ask for a form, businesses need to submit:
- Application for C/O Sample AK model has been fully and valid (Appendix VI);
- The AK C/O set has been completed, including one (01) original and two (02) copies;
- Customs declaration has gone through customs procedures;
- Commercial invoice;
- Bill of lading;
- If necessary, the C/O issuing organization may require the person who proposed the C/O grant to provide additional documents related to export products such as: Customs declaration of imported raw materials; Export licenses; Buying and Sales Contract; value added invoices in the country of domestic raw materials; Sample of raw materials or export products and other documents to prove the origin of export products.

Process of applying for form AK for businesses:
- Step 1: Declare the application of C/O Form AK online on the system of the Ministry of Industry and Trade at the address: http://ecosys.gov.vn
- Step 2: Submit a hard copy of the dossier at the Import -Export Management Department of the Ministry of Industry and Trade;
- Step 3: Officers at the agency conducting the application. If the application meets the conditions will be approved C/O Form AK. At the same time, the system will return information related to C/O;
- Step 4: Signing and approving C/O Form AK certification;
