Contents of C/O form B declaration
- Section 1: The applicant requesting C/O form B will declare his name and address information;
- Section 2: This section will declare the name and address of the consignee. If receiving goods as directed, this section will be declared as TO;

- Section 4: Enter name and address of the agency issuing C/O form B;
